^A High Deductible Health Plan (HDHP) is an insurance policy with a high deductible and usually a lower premium. It can be combined with a Health Savings Account to pay expenses toward the deductible with tax-free dollars. Required opening balance = $100.00. Interest compounded and credited monthly. Monthly Maintenance Fee of $4.95 will be imposed each monthly statement cycle if the average daily collected balance for the monthly statement cycle falls below $1,500.00. The average daily collected balance is calculated by adding the principal in the account for each day of the period and dividing that figure by the number of days in the period.
